Agreement

By registering a practice account or submitting cases through the GLAM Teeth portal, you agree to these Terms of Service. These terms govern the relationship between your dental practice and GLAM Teeth Dental Lab.

Account Eligibility

Accounts are available to licensed dental professionals and authorized staff of licensed dental practices. You must provide accurate registration information. GLAM Teeth reserves the right to refuse, suspend, or terminate accounts at its discretion.

Case Submissions

You are responsible for the accuracy of all prescription information submitted. You represent that all case files submitted comply with applicable privacy regulations. GLAM Teeth will fabricate restorations per the submitted prescription and cannot be held responsible for inaccuracies in the prescription.

Turnaround & Delivery

Turnaround times are estimates and not guarantees. Rush availability is subject to lab capacity. GLAM Teeth's liability for late delivery is limited to a credit toward future cases. We are not responsible for carrier delays beyond our control.

Remakes & Returns

Fabrication defects attributable to GLAM Teeth will be remade at no charge when reported within 30 days of delivery with supporting documentation via the portal. Cases remade due to prescription errors or changed requirements may incur a charge.

Limitation of Liability

GLAM Teeth's total liability shall not exceed the invoice value of the specific case in question. We are not liable for indirect, incidental, or consequential damages.
