Business Associate Agreement
GLAM Teeth Dental Lab acts as a Business Associate (BA) under the Health Insurance Portability and Accountability Act (HIPAA) when it receives, creates, or transmits Protected Health Information (PHI) on behalf of a Covered Entity (your dental practice).
When a BAA is Required
A signed BAA is required before any case containing PHI is submitted through the portal. GLAM Teeth provides a standard BAA to every approved practice account during the activation process. You will be prompted to review and execute the BAA electronically before your first case submission.
What the BAA Covers
Permitted uses and disclosures of PHI. Obligations and activities of GLAM Teeth as a Business Associate. Safeguards for PHI including administrative, physical, and technical safeguards. Reporting requirements for breaches or suspected breaches. Termination provisions and data return or destruction upon termination.
Requesting a BAA
Your BAA is automatically provided during account activation. If you need a copy of your executed BAA, or wish to review the BAA terms before registering, please contact legal@glamteeth.com.
Subcontractors
GLAM Teeth ensures that all subcontractors and vendors who may access PHI execute appropriate BAAs. This includes our cloud storage provider, email transaction service, and any other HIPAA-eligible service provider.
