Overview

GLAM Teeth Dental Lab ('Glam Teeth', 'we', 'our') takes the privacy and security of all data — including protected health information (PHI) associated with dental cases — extremely seriously. This policy describes how we collect, use, store, and protect information submitted through our website and Practice Portal.

Information We Collect

Practice registration information (name, NPI, address, email, phone). Case submission data including patient identifiers — preferably anonymous practice chart IDs. Uploaded case files (STL, PLY, OBJ, PDF, images). Usage and session data for portal security and audit logging. Billing and payment information (PCI-compliant, not stored directly by us).

How We Use Information

To process and fulfill dental laboratory cases. To communicate with authorized practice users regarding case status. To maintain audit logs for HIPAA compliance. To improve our services and portal experience. We do not sell, rent, or share practice or case information with third parties except as required to fulfill cases (e.g., shipping carriers) or as required by law.

HIPAA & PHI

We operate as a Business Associate under HIPAA. A Business Associate Agreement (BAA) is provided to every approved practice account. Case files and patient identifiers are stored in AES-256 encrypted object storage in HIPAA-eligible cloud infrastructure. PHI is never included in email or SMS notification bodies. All notification messages use case IDs only, with deep links back to the secure portal.

Data Retention

Case files and records are retained for a minimum of 7 years per dental industry standards. Practices may request data deletion for specific identifiers subject to applicable legal retention requirements. Contact our Privacy Officer at privacy@glamteeth.com for data requests.
